A. Mandatory Documents (For All Candidates)
All candidates must have the following documents ready before filling out the NEET UG 2026 application form:
1. Passport-size Photograph
Recent photograph
White background
Preferably taken within the last 6 months
2. Postcard-size Photograph
Same as the passport-size photograph
As per NTA specifications
3. Scanned Signature
Signed using blue or black ink
On plain white paper
4. Left and Right Hand Finger Impressions
Clear and properly scanned
5. Class 10 Mark Sheet
Used as proof of date of birth
6. Class 10 Passing Certificate
7. CGPA to Percentage Conversion Certificate (If applicable)
Required if marks are provided in CGPA format
8. Valid Photo Identity Proof (Any one of the following):
Aadhaar Card
Passport
PAN Card
Voter ID
DigiLocker ID
9. Present and Permanent Address Proof
B. Additional Documents (If Applicable)
The following documents are required only for candidates claiming
reservation or special status:
10. Category Certificate
SC / ST / OBC-NCL / EWS
Must be issued in the Central Government prescribed format
11. PwBD Certificate
For Persons with Benchmark Disabilities
Issued by an authorized medical authority
